- Tasks and hazards are determined for every life cycle phase of the application.
- Initial risk is assessed under the assumption that no risk reduction measures have been implemented for a given task/hazard pair.
- Required risk reduction measures are a function of the severity of initial risk.

You can then refer to this table to determine the level of each element in the chart.
